training and hands-on experience in infectious disease surveillance and emergency response, Ibrahim Seriki is the expert you need when developing a health plan for international borders—a critical but neglected area in global health security.\u003C/p>\n\n\n\n\u003Cp>After several years as a hospital-based physician, Dr. Seriki wanted to expand his impact from individuals to communities. He embarked on a two-year field epidemiology training program led by the U.S. Centers for Disease Control and Prevention (CDC) in Nigeria. This took him to 29 of the country’s 36 states, where he gained extensive field experience in infectious disease surveillance and response. Upon completion of the program, Dr. Seriki led a World Health Organization joint vaccination program in Nigeria focused on measles and meningitis.\u003C/p>\n\n\n\n\u003Cp>And then COVID-19 struck. With immunization programs paused and the pandemic calling for his expertise, Dr. Seriki headed to Lagos, the epicenter of COVID-19 in Nigeria. There, as a member of the COVID-19 response team, he supported the country’s surveillance efforts, including point-of-entry screening and testing.\u003C/p>\n\n\n\n\u003Cp>This led him to Jhpiego Sierra Leone, where he leads the CDC-funded Enhancing Global Health Security project. in terms of immigration—customs to check goods you are coming in with, and immigration to check whether you are eligible to come into the country. Port health services include screening passengers for disease as they enter or leave a country.\u003C/p>\n\n\n\n\u003Cblockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n\u003Cp>&#8220;But diseases do not recognize borders, crossing points or points of entry, so border health is not limited to screening, but also includes intensifying surveillance in the border communities and collaborating with neighboring countries on public health functions&#8221;. — Ibrahim Seriki\u003C/p>\n\u003C/blockquote>\n\n\n\n\u003Cp>It also includes risk communication, infection prevention and control, laboratory capacity for testing at the point of entry, and vaccination. As the International Health Regulations (IHR) (2005) outline, the point is to prevent the international spread of diseases. [The IHR (2005) is a legally binding agreement among 196 countries that defines how countries should respond to public health emergencies of international concern.]\u003C/p>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">\u003Cstrong>Q: How does health security at land borders differ from health security at airports or seaports?\u003C/strong>\u003C/h5>\n\n\n\n\u003Cp>IS: The vast majority of travelers are crossing through the land borders—at both designated [those that meet IHR core capacities] and undesignated ground crossing points—not the airport. Only people who have money can afford to fly.&nbsp;\u003Cstrong>Ground crossing points should get more attention\u003C/strong>&nbsp;when it comes to response. The risk of spread is evident from the Ebola outbreak in 2014/2015. The outbreak started in Guinea and affected all the neighboring countries because the communities are so interconnected, with vast population movement. The majority of people who pass through the crossing points live within the border communities, especially those informal crossing points that are not manned. So it’s necessary to intensify surveillance in the nearby health facilities—and ensure the supply of medical consumables.\u003C/p>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">\u003Cstrong>Q: What are the main issues to consider when developing a border security plan?\u003C/strong>\u003C/h5>\n\n\n\n\u003Cp>IS: For border health generally, we are governed by the International Health Regulations. At the same time, we try to tailor the response plan to the locally accepted way of doing things. Then, of course,&nbsp;\u003Cstrong>communication is key\u003C/strong>. You need to communicate to travelers why you’re doing this, what are the risks, what diseases are endemic in the visiting country. And there should be community engagement with border communities, because in some places people are crossing without [border] personnel for screening. Community stakeholders can play a role in supporting these unmanned crossing points.\u003C/p>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">\u003Cstrong>Q: How do you prepare health workers and officials from different countries to work together and respond to an actual health emergency?\u003C/strong>\u003C/h5>\n\n\n\n\u003Cp>IS: You have to maintain collaboration over time. If there’s&nbsp;\u003Cstrong>strong collaboration between countries\u003C/strong>, maybe through a formal agreement and information-sharing mechanism, then there can be a joint response because they’re used to doing things together. We initiated some level of collaboration during a regional tabletop exercise [a desktop simulation of an emergency response, such as a Lassa fever outbreak]. Our colleagues from Liberia and Guinea came to act as observers and evaluators, and we also went to Liberia to do the same. We’ve built some connections. To sustain that connection, we have to continue to do activities together. We’ve had regional meetings, as well, with the four countries in the Mano River Union region (Liberia, Guinea, Sierra Leone and Côte d’Ivoire) and binational meetings with Liberia.\u003C/p>\n\n\n\n\u003Cp>We need joint cross-country plans, like the one we developed for Sierra Leone and Liberia during one of our cross-border meetings. Prediction and prevention of outbreaks will only be relevant if countries respond as one.\u003C/p>\n\n\n\n\u003Cp>\u003Cstrong>Every outbreak is a lesson\u003C/strong>. After the Ebola outbreak, diverse country coordination has been strengthened. Countries have learned how to mount an effective response to public health emergencies. The COVID-19 pandemic also provided an opportunity for the health security to be strengthened, despite language differences. For instance, Guinea has had recent disease outbreaks (especially viral hemorrhagic fevers) in the region. And they didn’t disappoint. They shared a lot of best practices. That’s the idea of these exercises—simulating a scenario and everybody gets to talk about how they would respond.\u003C/p>\n\n\n\n\u003Cp>You can never stop preparing. It’s not cheap, but if you prepare very well, it will save you a lot more money if there is a pandemic or outbreak—and not just money, but lives lost. Preparedness helps reduce the money you’re spending when there’s an emergency. That’s why we have partnerships like \u003Ca href=\"https://www.worldbank.org/en/programs/financial-intermediary-fund-for-pandemic-prevention-preparedness-and-response-ppr-fif/overview\">The Pandemic Fund\u003C/a>. The World Bank, Global Fund, everybody now wants to invest in preparedness.\u003C/p>\n\n\n\n\u003Ch5 class=\"wp-block-heading\">\u003Cstrong>Q: What other actions are you taking to support border health?\u003C/strong>\u003C/h5>\n\n\n\n\u003Cp>IS: Something we are doing uniquely here is&nbsp;\u003Cstrong>population movement mapping in land border\u003C/strong>&nbsp;districts to identify areas with risk of disease transmission. This is a mixed-method study with qualitative focus group discussions and spatial analysis to identify where people frequent, including their point of entry and the reason for the visit. It could be for health care, for religious purposes, for schooling, or for markets. When we identify which areas are visited the most, we plan in terms of what to do if there’s an outbreak in the neighboring country, and make plans to allocate resources accordingly. During preparedness or silent periods, we can recommend that those places be strengthened for surveillance as well as risk communication. You can strengthen surveillance at the health facility. You can strengthen risk communication at the schools, at the market and at religious sites. You can also target the health facility for increased infection prevention supplies because they may be at more risk.\u003C/p>\n\n\n\n\u003Cp>We are also supporting the Ministry of Health to develop&nbsp;\u003Cstrong>a national border health policy\u003C/strong>. While the public health emergency response plan is meant for emergency settings, the border policy will guide both routine and emergency settings. The policy, based on the International Health Regulations, will detail the governance, components of the port health system and roles and responsibilities of the different sectors working in the national border health system. I don’t think any other country has this policy. Antimicrobial resistance (AMR), when bacteria, viruses, fungi, and parasites no longer respond to antimicrobial medicines, is a growing crisis worldwide, and Pakistan is among the countries most affected. But what does this mean? And what can we do to stop it?\u003C/p>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>What is AMR?\u003C/strong>\u003C/h2>\n\n\n\n\u003Cp>Antimicrobial resistance (AMR) is a major global health threat that occurs when infections no longer respond to the medications used to treat them, making them harder to treat and increasing the risk of severe illness, disability, and death. According to \u003Ca href=\"https://www.who.int/news-room/fact-sheets/detail/antimicrobial-resistance#:~:text=Key%20facts,4.95%20million%20deaths%20(1).\">global estimates\u003C/a>, AMR was directly responsible for approximately \u003Cstrong>1.27 million deaths\u003C/strong> in 2019, with millions more affected by resistant infections due to limited treatment effectiveness. Tragically, one child dies every three minutes from multidrug-resistant organism sepsis—when bacteria responsible for an infection are resistant to common antibiotics. In Pakistan, \u003Ca href=\"https://asm.org/magazine/2023/fall/exploring-a-one-health-approach-to-amr-in-pakistan#:~:text=The%20Status%20of%20AMR%20in,serotype%20Typhi%20(XDR%20typhoid).\">AMR was the third leading cause of death in 2019\u003C/a>, with alarmingly high resistance levels observed in several common disease-causing organisms. These include \u003Cem>Escherichia coli\u003C/em> (E. coli), which causes gastrointestinal infections; \u003Cem>Klebsiella pneumoniae\u003C/em> (K. pneumoniae), a major cause of pneumonia and Methicillin-resistant \u003Cem>Staphylococcus aureus\u003C/em> (MRSA), for bloodstream infections.\u003C/p>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Why Should We Care?\u003C/strong>\u003C/h2>\n\n\n\n\u003Cp>As researchers, we’re dedicated to finding solutions to this growing threat. When these bacteria become resistant to treatment, even routine infections can become life-threatening.\u003Cem> \u003C/em>In the United States, over 2.8 million antibiotic-resistant infections occur annually, causing more than 35,000 deaths according to the \u003Ca href=\"https://stacks.cdc.gov/view/cdc/82532\">Centers for Disease Control and Prevention (CDC)\u003C/a> and in Pakistan Antimicrobial resistance contributes to 200,000 annual deaths according to the \u003Ca href=\"https://www.emro.who.int/pak/pakistan-news/antimicrobial-resistance-contributes-to-200-000-annual-deaths-in-pakistan-who-and-the-nih-call-for-action.html\">World Health Organization\u003C/a>. A \u003Ca href=\"https://documents1.worldbank.org/curated/en/455311493396671601/pdf/executive-summary.pdf\">World Bank report\u003C/a> warns that AMR could cost up to $1 trillion to the annual global gross domestic product (GDP) by 2050. In Pakistan, surveillance efforts have expanded significantly, as seen in the \u003Ca href=\"https://www.nih.org.pk/public/wp-content/uploads/2024/09/National%20AMR%20Surveillance%20Report%20year%202021-2022.pdf\">National AMR Surveillance Report 2021–2022\u003C/a>, which covered over 229,000 bacterial isolates across 26 sentinel sites (a network of specific locations that collect detailed health data to monitor diseases and trends in a larger population). However, more inclusive approaches are still needed. For instance, the report lacks gender-disaggregated data, limiting the ability to understand how AMR affects women and children differently. Without this level of detail, targeted interventions to protect vulnerable groups will remain challenging to design and implement.\u003C/p>\n\n\n\n\u003Cp>This is exactly the gap Jhpiego’s recent consultations sought to address.\u003C/p>\n\n\n\n\u003Ch2 class=\"wp-block-heading\">\u003Cstrong>Recent Momentum in Pakistan: A Gender-Responsive Approach to AMR\u003C/strong>\u003C/h2>\n\n\n\n\u003Cp>Recognizing these critical gaps in gender data and surveillance, Jhpiego recently gathered experts to find a solution. A national roundtable discussion on Gender Inequalities and AMR was held, attended by policymakers, clinicians, laboratory experts, veterinarians, academics, and public health leaders. Tigistu Adamu Ashengo, Chief Medical Officer at Jhpiego, presented insights on AMR and infectious diseases, while Elizabeth Arlotti-Parish, the former Principal Technical Advisor for Gender at Jhpiego, shared a powerful expert perspective. Their contributions sparked thoughtful dialogue among stakeholders, including National Institutes of Health\u003Cstrong> \u003C/strong>(NIH) leadership, the Fleming Fund, Pathfinder, and other partners.\u003C/p>\n\n\n\n\u003Cp>Building on the roundtable, Jhpiego, in collaboration with the \u003Cstrong>NIH\u003C/strong> and the \u003Cstrong>Fleming Fund,\u003C/strong> led a \u003Cstrong>Consultative Workshop\u003C/strong> to begin drafting \u003Cstrong>Pakistan’s first Gender-Specific AMR Strategy\u003C/strong>. Using WHO’s key thematic recommendations, participants worked across four thematic areas—\u003Cstrong>Human Health, Animal Health, Agriculture, and Climate\u003C/strong>—to design strategic actions, define roles, and set realistic timelines. Facility upgrades and staff training are necessary to protect patients.\u003C/li>\n\n\n\n\u003Cli>\u003Cstrong>Surveillance Data Gaps\u003C/strong>\u003Cbr>Without gender-disaggregated AMR data, the invisible burden on women cannot be addressed. Strengthening data systems is vital for equitable policy response.\u003C/li>\n\n\n\n\u003Cli>\u003Cstrong>Hospital-Acquired Infections (HAIs)\u003C/strong>\u003Cbr>Unnecessary procedures and poor aseptic practices during childbirth and surgeries increase resistant infections. Strengthening IPC protocols can save lives.\u003C/li>\n\n\n\n\u003Cli>\u003Cstrong>Policy and Governance\u003C/strong>\u003Cbr>Weak enforcement, limited female representation in leadership, and unregulated antibiotic use in agriculture drive resistance. Strengthening policy, updating the National Action Plan, and integrating AMR into medical and veterinary training are critical.\u003C/li>\n\n\n\n\u003Cli>\u003Cstrong>Private Sector Engagement\u003C/strong>\u003Cbr>The private sector provides most outpatient care in Pakistan, yet it is often not integrated into AMR governance. Collaboration and incentives can close this gap.\u003C/li>\n\n\n\n\u003Cli>\u003Cstrong>Diagnostics and Surveillance\u003C/strong>\u003Cbr>Insufficient diagnostic capacity leads to guesswork, fueling antibiotic misuse. Pakistan must expand laboratory networks and invest in rapid, affordable diagnostic tools.\u003C/li>\n\u003C/ol>\n\n\n\n\u003Cp>\u003Cstrong>Community Level Challenges\u003C/strong>\u003C/p>\n\n\n\n\u003Col start=\"1\" class=\"wp-block-list\">\n\u003Cli>\u003Cstrong>Prevention Challenges\u003C/strong>\u003Cbr>Rural populations face higher exposure due to livestock handling, caregiving roles, and limited access to health care. Women in particular are disproportionately exposed yet underrepresented in AMR decision-making. Empowering women as community educators and frontline responders is essential.\u003C/li>\n\n\n\n\u003Cli>\u003Cstrong>Overuse and Misuse of Antimicrobials\u003C/strong>\u003Cbr>Easy, prescription-free access encourages misuse, especially among women and children who may delay formal health care due to social restrictions. Stewardship programs and dispensing regulations are needed.\u003C/li>\n\n\n\n\u003Cli>\u003Cstrong>Community Engagement\u003C/strong>\u003Cbr>Harmful traditional practices and low awareness perpetuate infection risks. access to effective antibiotics thereby mitigating the growing threat of antimicrobial resistance.&nbsp;In partnership with eHealth Africa, Jhpiego Nigeria submitted an innovative digital solution called Com-WATCH which integrates data-driven technology to manage inventory control and spot substandard and falsified antibiotics at the community level across Nigeria.&nbsp;\u003C/p>\n\n\n\n\u003Cp>Jhpiego was initially shortlisted from 171 applications across 51 countries for The Trinity Challenge on Community Access to Effective Antibiotics and was chosen to share the £1 million prize fund alongside the University of Notre Dame’s PADS initiative. With £500,000, Jhpiego and partner eHealth Africa aim to replace fragmented, paper-based inventory&nbsp;systems with affordable, technology-driven solutions that integrate stock control and surveillance mechanisms to detect substandard and falsified antibiotics and ensure the availability of antibiotics.&nbsp;\u003C/p>\n\n\n\n\u003Cp>Over the next two years, Com-WATCH will deploy a community-powered, AI-enabled surveillance and inventory management tool. Through a simple app, SMS, and chatbot, users can verify the authenticity of antibiotics for human and animals using image recognition matched against Nigeria’s regulatory database. At the same time, vendors can track inventory, monitor expiry dates, and receive alerts before stock runs out, which can also operate offline. This year, the stakes were higher than ever, with a staggering 147% increase in rainfall, elevating the risk of flooding to unprecedented levels.\u003C/p>\n\n\n\n\u003Cp>Identifying two blocks as highly susceptible, a targeted and comprehensive initiative was launched to fortify emergency preparedness. Engaging 62 health sub-centres and mobilizing 405 ASHAs, the intervention became a community-wide effort. the Storm: Prepared for Public Health Emergencies","\n\u003Ch2 class=\"wp-block-heading\">Rumblings of a brewing storm\u003C/h2>\n\n\n\n\u003Cp>At a morning meeting in early September 2022, as department heads at Mubende Regional Referral Hospital in central Uganda reported out, it became clear that something was very wrong. Patients from each unit were showing up with similar symptoms: high fever, convulsions, diarrhea and blood-stained vomit. And all of the patients were from the same location.\u003C/p>\n\n\n\n\u003Cp>A surveillance team of 12 from the Ministry of Health (MOH) was sent to the area to gather more information. There, the team discovered that seven people had died showing the same symptoms as those at the hospital. They collected samples that were transported to the Uganda Virus Research Institute in Entebbe for testing, more than a three-hour drive from Mubende Regional Referral Hospital. The samples tested positive for Ebola virus disease.\u003C/p>\n\n\n\n\u003Cp>On September 23, 2022, the government officially declared an outbreak of Ebola virus disease in Mubende District. The district would be the initial epicenter of this outbreak, the first Ebola outbreak the country had seen in a decade.\u003C/p>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">In the thick of the storm: Quick actions and steps taken\u003C/h3>\n\n\n\n\u003Cp>Swift action followed. The MOH set up a coordination center at Mubende Regional Referral Hospital, reassigned medical teams to the hospital and immediately issued personal protective equipment, handwashing stations, medicines and supplies. Mubende town was locked down.\u003C/p>\n\n\n\n\u003Cp>Nurse Halima Adam was working at the accidents and emergencies unit when she was moved to the Ebola treatment unit (ETU), a temporary canvas structure set up within the Mubende Regional Referral Hospital compound. “As soon as the outbreak was declared, I called in six nurses to work fully at the ETU. A duty roster was made, and we coordinated procurement of medical items,” she said.\u003C/p>\n\n\n\n\u003Cp>By the second day, eight patients presenting with Ebola symptoms were isolated at the ETU.\u003C/p>\n\n\n\n\u003Cp>One evening as her shift ended, Juliet Nayiga, a laboratory technician at the hospital, was called to collect blood samples from the patients in the ETU, a daunting task under the circumstances. “I remember it was dark and the lights had not been set up at the ETU. I was panicking. We created storage areas for the personal protective equipment and handwashing items. We learned how to set up the red and green zones to manage and treat confirmed cases,” Halima said.\u003C/p>\n\n\n\n\u003Cp>Hospital staff later joined emergency medical teams and responders from various regions at a training conducted by the MOH, Doctors Without Borders, World Health Organization (WHO), UNICEF and Jhpiego to strengthen the skills of health workers. Jhpiego-trained staff were part of the responders seconded to the national incident management team, which was responsible for oversight during the Ebola outbreak, as pillar administrators to cover logistics, surveillance, risk communication, laboratory work, strategic information research and innovation.\u003C/p>\n\n\n\n\u003Cp>A total of 168 health workers were identified from three regional hospitals—Mubende, Gulu and Naguru—to be part of the training on infection prevention and control procedures, clinical care and psychosocial support. All affected districts enhanced their surveillance systems to ensure that all contacts would be identified, documented and followed up with for 21 days.\u003C/p>\n\n\n\n\u003Cp>“It was tough at the beginning, but as time went by, it got better. Building on our previous experience from COVID-19 and the training by WHO, infection prevention and control processes became easier,” Halima said.\u003C/p>\n\n\n\n\u003Cp>Uganda had a total of 164 Ebola patients in nine districts (142 confirmed cases and 22 probable), with 55 confirmed deaths. Mubende District reported the highest numbers, with 64 confirmed cases and 29 deaths due to Ebola virus disease. The last confirmed case was reported on November 14—exactly 56 days after the first confirmed case.\u003C/p>\n\n\n\n\u003Ch4 class=\"wp-block-heading\">\u003Cstrong>Calm after the storm\u003C/strong>\u003C/h4>\n\n\n\n\u003Cp>“Working as a team—the health care workers at the hospital, MOH and partners—was a critical aspect that contributed to managing the Ebola outbreak in Uganda,” says Deogratius Kamulegeya, Jhpiego’s technical officer in Uganda and part of the Mubende response team.\u003C/p>\n\n\n\n\u003Cp>“Today, Jhpiego is working with the MOH through the Enhancing Global Health Security project, funded by the U.S. Centers for Disease Control and Prevention, to develop an outbreak management support manual and set up a central data warehouse for all surveillance data for epidemic intelligence to support quick detection of outbreaks in Uganda,” said Peter Muwereza, Jhpiego’s Program Manager for the Enhancing Global Health Security project in Uganda.\u003C/p>\n\n\n\n\u003Cp>To this end, Jhpiego seconded six staff members to the MOH and is supporting the MOH and other implementing partners to develop tools such as data visualization and analytics, ensuring operationalization of the data warehouse and integrating data with the electronic medical records system.\u003C/p>\n\n\n\n\u003Cp>Reflecting on the outbreak, Halima said: “I would have left when it got worse. class=\"wp-block-heading\">A tick\u003C/h3>\n\n\n\n\u003Cp>A Pakistani cattle herder, too sick to mind his livestock, went to a hospital in Balochistan, the country’s largest but least populated province located in the southwest, bordering Afghanistan. The medical staff initially suspected a snake bite; the herder was vomiting, had a high fever and an abnormal blood clotting profile. The health care provider tending to the herder didn’t wear goggles, a mask or gloves. Having been exposed to the patient’s bodily fluids, he rinsed his hands with just water and then sat down for dinner with his colleagues. Within days, that health care provider and others began suffering with shuddering chills and other severe symptoms of viral infection; one started bleeding from the nose and various mucous membranes.\u003C/p>\n\n\n\n\u003Cp>Misdiagnoses and ineffective treatment complicated the situation. The cattle herder recovered but a week later, three health care workers died, including a doctor who had dined with colleagues and friends.\u003C/p>\n\n\n\n\u003Cp>Now, two of the deceased doctor’s colleagues are sick. Although described as a disease of unknown origin at the start, there is new confirmation by the Provincial Public Health Laboratory of Crimean-Congo hemorrhagic fever (CCHF), an endemic viral illness transmitted to humans by ticks or by infected animals, such as livestock. Human-to-human transmission occurs through direct contact with the blood and other body fluids of infected persons.\u003C/p>\n\n\n\n\u003Cp>This is where I come in. A Pakistani infection prevention and control (IPC) specialist working closely with the Ministry of Health on global health security issues, I was summoned to investigate and support the management of this outbreak. My charge is to help prevent the further spread of an illness that already has claimed the lives of three Pakistanis after it crossed the Pakistan-Afghanistan border and jumped from cattle to humans. Some sit on a sofa sandwiched between the two hospital beds occupied by their sick colleagues. All had eaten and socialized with the doctor who died the previous day from massively severe bleeding.\u003C/p>\n\n\n\n\u003Cp>They sip tea and share fruit.\u003C/p>\n\n\n\n\u003Cblockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n\u003Cp>&#8220;I think I am in the wrong room,” I apologize.&#8221; — \u003Cem>Dr. Sobhan Qadir\u003C/em>\u003C/p>\n\u003C/blockquote>\n\n\n\n\u003Cp>“No, No,” they insist, welcoming me to enter. “This is the room of the doctors [who are now patients with CCHF] and we are here to have fruit with them.”\u003C/p>\n\n\n\n\u003Cp>They offer me tea. No one wears gloves.\u003C/p>\n\n\n\n\u003Cp>They smile. No one wears a mask.\u003C/p>\n\n\n\n\u003Cp>“I’m OK,” I say. “I’m good.”\u003C/p>\n\n\n\n\u003Cp>Actually, what I am is shocked. Even after losing their friend, a fellow doctor, basic transmission-based precautions are not being followed.\u003C/p>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">IPC\u003C/h3>\n\n\n\n\u003Cp>I realized then the core reason behind this outbreak: Even if there was adequate PPE for everyone to wear routinely, at all times, they wouldn’t be wearing it here and now. And they plan to leave this “isolation” room soon and head back to their patients and families. As a former member of the Young Doctor Association in Pakistan from this same region, I can assure you that neither ignorance nor arrogance nor denial are at issue. Doctors and surgeons in traditional hierarchical health care roles risk mistaking the implementation of IPC as someone else’s concern; notably as the duty of nurses who provide direct patient care. Providers in health care facilities and communities too often lack an understanding of the importance of the basics of preventing and controlling infections.\u003C/p>\n\n\n\n\u003Cp>More than Ebola, Zika and MERS (Middle East Respiratory Syndrome), COVID-19 showed us how failing to change a seemingly simple behavior can have alarming results. Not washing hands, not masking and not isolating caused preventable illness, suffering, death and economic havoc.\u003C/p>\n\n\n\n\u003Cp>Certain diseases, like CCHF and COVID-19, require specific IPC responses that relate to the mode of transmission. There also are standard precautions that always apply, such as simple hand hygiene, protective equipment and waste management.\u003C/p>\n\n\n\n\u003Cp>In this case, the Provincial Health Department took a systematic approach to control the outbreak with Jhpiego’s technical guidance and recommendations. Awareness, education, contact tracing, isolation measures and IPC precautions were instituted in the health care facility. But the lesson was clear: Be proactive in preventing infections and have at the ready a robust response by coordinating agencies, including those monitoring animal health. Don’t wait until a public health outbreak occurs.\u003C/p>\n\n\n\n\u003Cp>Infection prevention and control is the cornerstone of global health security. Necessary as national documents and global guidelines are, all is for naught if proper implementation of IPC and related measures aren’t happening on the ground, not only in tertiary hospitals but also in provincial country hospitals and clinics.\u003C/p>\n\n\n\n\u003Cp>I’ve seen firsthand how a tiny tick and a sip of tea stand to challenge our most sophisticated data-led measures to ensure global health security. That’s why we need enough equipment and enhanced education and training, cultural awareness and regulations and a commitment to use them. The goal is to have PPE in place and IPC practices operational before we discover that a disease variant is just waiting for someone—anyone—to offer a cup of tea, believing using one’s own cup is protection enough against the next pandemic.\u003C/p>\n\n\n\n\u003Cp>Enhancing Global Health Security, implemented by Jhpiego in 21 countries, strengthens IPC, diagnostics, surveillance and responses to public health threats by supporting nations’ health care systems. />\u003C/figure>\n\n\n\n\u003Cp>The biobank. It’s an essential concept for our global health security systems and preventing the next disease outbreak. Unfortunately, it’s overlooked in most health care systems as disease response has become the primary focus over the past few years.\u003C/p>\n\n\n\n\u003Cp>A repository or collection of biological samples, a biobank aims to preserve biological resources—from humans, plants, animals and other aspects of nature—in optimal condition so they are available to the scientific community for the advancement of research and knowledge worldwide. This helps facilitate the development of innovative medicines, vaccines and diagnostic tests, which strengthens epidemiological surveillance and advances genomic research, such as discovering treatments for different types of cancer.\u003C/p>\n\n\n\n\u003Cp>Samples donated by clients with breast cancer, for example, have been used to identify therapeutic targets and help predict which groups of people would (or would not) benefit from a particular treatment, thus providing critical information for the development of a treatment strategy. These client samples are an indispensable resource for cancer research and an unparalleled asset for the development of more effective personalized treatments for many diseases.\u003C/p>\n\n\n\n\u003Cp>Biological specimens have been collected in biobanks for decades, but it was only in the 1990s that biobanks were established more systematically, predominantly in high-income countries. Today, despite being the second most populous continent with incredible biodiversity, Africa contributes very little to the advancement of research for treatment solutions. Biological data banks would help make scientific research more inclusive and wider reaching. The&nbsp;\u003Ca href=\"https://bbsa.org.za/\">Biodiversity Biobanks South Africa\u003C/a>&nbsp;project, for example, has more than half a million items in various biobanks across the country, but it is&nbsp;\u003Ca href=\"https://www.thelancet.com/journals/lancet/article/PIIS0140-6736(19)31614-9/fulltext\">one of only a few country-based biobanks\u003C/a>&nbsp;on the continent, all of which contribute only&nbsp;\u003Ca href=\"https://nam11.safelinks.protection.outlook.com/?url=https%3A%2F%2Fwww.esmo.org%2Foncology-news%2Fthe-establishment-of-pan-africa-s-first-biobank&amp;data=05%7C02%7CKatherine.Seaton%40jhpiego.org%7Cb4f086b3f8ff48ffc97908dc76958083%7C26ef7fd22a7f4135a2e4de9acf168b2a%7C0%7C0%7C638515631968696047%7CUnknown%7CTWFpbGZsb3d8eyJWIjoiMC4wLjAwMDAiLCJQIjoiV2luMzIiLCJBTiI6Ik1haWwiLCJXVCI6Mn0%3D%7C0%7C%7C%7C&amp;sdata=othCbg4eQhmhCuxMySSPszDhj3%2F66uXw6zfZi9b2B8g%3D&amp;reserved=0\">2% of the data used in genome-wide studies\u003C/a>.\u003C/p>\n\n\n\n\u003Cp>In Burkina Faso, local capacity for the conservation of biological samples has been weak. It has been hard for the country to conserve biological resources at both the technical and biosecurity levels in the regions because of inadequate conditions. Burkina Faso is left to export large quantities of these samples to high-income countries with biobanks that have more diagnostic tools and meet international technical standards.\u003C/p>\n\n\n\n\u003Cp>But that is about to change.\u003C/p>\n\n\n\n\u003Cp>When COVID-19 spread around the world, it highlighted the importance of the biobank as a tool to prevent future public health emergencies. In February 2022, Burkina Faso initiated the creation of a national biobank using their virtual electronic pathogen registry, a database of biological resources stored in the country’s nine national reference laboratories (tuberculosis, antimicrobial resistance, meningitis, HIV, human papillomavirus, hemorrhagic fever, measles, rotavirus and influenza). But setting up the physical biobank has not been easy. Jhpiego has been supporting Burkina Faso’s National Lab service since 2018 through a project funded by the U.S. Centers for Disease Control and Prevention. This includes training staff, conducting assessments, drafting procedures, monitoring virtual data and purchasing and installing equipment according to standards. In June 2024, Burkina Faso inaugurated a physical biobank structure on a temporary site, with plans to move it to a permanent location once funds are secured.\u003C/p>\n\n\n\n\u003Cp>Successfully setting up a central biobank requires overcoming financial, infrastructural, organizational and documentation difficulties. One of the first requirements is for all academic institutions, research institutes and reference laboratories in the country to establish a centralized inventory system for samples of interest. If resources are limited, electronic pathogen registries, with a focus on supervision and the development of a virtual biobank interface, can replace a physical biobank until a permanent, physical one can be completed.\u003C/p>\n\n\n\n\u003Cp>Biobanks play a crucial role in the development of many health care approaches and treatments. They inform ways to reduce behaviors that put one’s health at risk, ensure universal access to medical coverage and health services, support research and development of vaccines and medicines and improve management of health risks. This underscores the importance of biobanks to global health systems and their lifesaving potential. According to the WHO, over 300 million people are living with hepatitis B and C globally, and over 1.3 million die each year from the complications of the disease. Despite the availability of tools and solutions to efficiently tackle viral hepatitis, most countries are far from reaching the 2030 goals to eliminate the virus-caused disease due to programmatic, policy and access barriers.\u003C/p>\n\n\n\n\u003Cp>To stimulate discussions and actions from global stakeholders, share evidence and experiences, and disseminate best practices in the fight against viral hepatitis, the World Hepatitis Alliance, the World Health Organization, and the Government of Portugal hosted a Summit in April 2024 in Lisbon. Over 600 delegates from 95 countries joined in person and virtually, including Jhpiego representatives who shared their work and learnings on efforts to strengthen health services to address Hepatitis C in Nigeria.\u003C/p>\n\n\n\n\u003Cp>Nigeria is one of 10 countries that bears the biggest burden of hepatitis B and C.&nbsp;\u003Ca href=\"https://www.afro.who.int/countries/nigeria/news/nigeria-boosting-viral-hepatitis-awareness-and-treatment-0#:~:text=More%20than%2090%20million%20people,%2C%202018%20(NAIIS%202018).\">Over 20 million people live with hepatitis B, C, or both\u003C/a>&nbsp;in the country but more than 80% of them do not know their status. Research conducted by the Unitaid-funded STAR Phase 3 initiative and presented by Chidinma Umebido, a senior Monitoring, Evaluation and Learning advisor for Jhpiego Nigeria, found that advocacy and awareness, implementation of simplified service delivery models, peer support and stigma reduction, and tailored health services are key components to improving hepatitis B and C health care in the country.\u003C/p>\n\n\n\n\u003Cp>To strengthen these components and integrate hepatitis B and C screening into existing programs, collaboration with federal and state Ministries of Health and local communities is crucial (such as HIV, key populations and maternal and newborn health) as is expanding access to testing and treatment, specifically Hepatitis C self-testing. The surge in frequency and intensity of extreme weather events, including hurricanes, floods, wildfires, and heatwaves, directly impacts public health leading to displacement, injuries, and loss of life. Many of the worst impacts are felt by populations that are already suffering because of multiple health burdens and vulnerabilities combined with poverty and socioeconomic exclusion. These vulnerable populations face compounding challenges in accessing clean water, shelter, and medical care thus propagating the emergence and rapid spread of disease.\u003C/p>\n\n\n\n\u003Cp>The World Health Organization estimates that climate change will cause an additional 250,000 deaths annually between 2030 and 2050. This is a staggering figure.\u003C/p>\n\n\n\n\u003Cp>The challenges posed by the intersection of climate change and public health are immense indeed, but in facing them, we have also gained valuable insights. The COVID-19 pandemic highlighted the importance of preparedness, response, resilience, and recovery in the face of health crises, with lessons that are applicable to the broader context of climate change. The interconnectedness of our global community was a crucial lesson. Climate change knows no borders, emphasizing the necessity of collaboration and shared responsibility for the well-being of humanity.\u003C/p>\n\n\n\n\u003Cp>So how can we start to think about possible actions? The first step is to identify and map vulnerability. Climate health impacts will not affect us all in the same way. We know that vulnerability to climate health-related consequences will be influenced by our geography, our demographics, our biology and health status, the political and social economic status in which we live, as well as the strength of our health systems.\u003C/p>\n\n\n\n\u003Cp>We have a chance now, in the face of a global threat, to truly reorient health action and service provision around the needs of the most vulnerable. There are opportunities to transform our healthcare systems through sustainable practices, investment in renewable energy, and integration of climate resilience into infrastructure to build a more robust and adaptive healthcare system.\u003C/p>\n\n\n\n\u003Cp>For example, in Bangladesh, a low-lying country vulnerable to sea level rise, flooding, and cyclones, the government has implemented a number of initiatives to enhance the resilience of its healthcare system. These include the construction of climate-resilient health facilities, the development of early warning systems, the training of health workers and the provision of mobile health services. In Nepal, a mountainous country prone to landslides, avalanches and flooding, the government has also taken steps to improve the climate resilience of its healthcare system. These include the establishment of a climate and health unit, integration of climate change into health policies and plans, and promotion of community-based climate adaptation.\u003C/p>\n\n\n\n\u003Cp>Promoting public awareness and education through risk communication and community engagement will equip communities to adapt to changing environmental conditions, protect against health threats and advocate for policies addressing both climate change and public health.\u003C/p>\n\n\n\n\u003Cp>In Bhutan, the government has undertaken a sweeping nationwide campaign aimed at educating the public about climate change and its implications for health.
