They administered important vaccinations given during pregnancy such as the Tetanus vaccine, and provided me with essential education on nutrition, birth preparedness, and the early detection of danger signs, transforming my pregnancy into a safe and informed journey. Jhpiego is leading AMPLI-PPHI in partnership with PATH and the International Federation of Gynaecology and Obstetrics (FIGO), which aims to generate evidence and learning, create an enabling environment, and prepare the market to enable broad uptake of newly recommend PPH products – heat-stable carbetocin to prevent PPH, calibrated drapes to detect PPH, tranexamic acid as part of the MOTIVE bundle to treat PPH and misoprostol for administration in home births across low- and middle-income countries. Her husband, Martudilah, drove her by motorbike on the 30-minute journey to their public hospital where she gave birth to a premature, underweight boy, Edgar.\u003C/p>\n\n\n\n\u003Cp>Hamriani was much further along—38 weeks. She awoke one night with a throbbing headache and realized she was having contractions. In a borrowed car, her husband took her to their health center where midwives quickly identified the warning signs for pre-eclampsia, a life-threatening hypertensive disorder.\u003C/p>\n\n\n\n\u003Cp>Desi was 40 weeks pregnant and ready to have her baby. Throughout Indonesia, complications at birth continue to threaten the lives of women and their babies. The number of maternal and newborn deaths in Indonesia remains unacceptably high and, by some indications, has increased. From 2007 to 2012, for example, Indonesia’s estimated maternal mortality ratio increased from 228 to 359 deaths for every 100,000 births.\u003C/p>\n\n\n\n\u003Cp>These three women, fortunately, survived pregnancy and childbirth with the help of the Expanding Maternal and Neonatal Survival (EMAS) program that was launched by the government of Indonesia through the support of the U.S. Agency for International Development (USAID). EMAS, a program led by Jhpiego, works across the health system from village center to hospital to strengthen business practices, supervision and oversight of health service providers and the skills of individual midwives.\u003C/p>\n\n\n\n\u003Cp>The EMAS program helps train health workers to recognize and manage complicated labors like those Desi and Hamriani experienced. Twice a month, the midwives and nurses at Hamriani’s health center practice emergency drills to ensure that they’re always confident and ready to take action when the need arises. In fact, the very same day that Hamriani arrived at her health center with pre-eclampsia, midwives there had just completed a skill-building session on addressing pregnancy-related high blood pressure and its impact on mother and child. With skills fresh on their minds, the midwives worked together as a team, each managing a different aspect of Hamriani’s difficult labor. transmission rises sharply during the rainy season, which runs from June to October. Increased rainfall creates stagnant water that serves as breeding sites for Anopheles mosquitoes that transmit malaria. Climate factors such as higher humidity and temperature further support mosquito survival and parasite development, contributing to a surge in cases during this period.\u003C/p>\n\n\n\n\u003Cp>“When the heavy rains swell the lake separating their village from mine, parents in Ildong-Souaye worry about how their children will receive antimalarial medication,” said Jeanette.\u003C/p>\n\n\n\n\u003Cp>To address this risk, Jhpiego, with funding from GiveWell and other donors, supported the Ministry of Public Health’s seasonal malaria chemoprevention (SMC) campaign in 2025. Each year, trained health workers and community mobilizers go door to door to administer a preventive antimalarial combination—sulfadoxine-pyrimethamine plus amodiaquine (SPAQ)—to children ages 3 to 59 months. The treatment is given monthly for four or five cycles, depending on local transmission patterns, ensuring protective drug levels throughout the peak season.\u003C/p>\n\n\n\n\u003Cp>Reaching these children often requires extraordinary effort. Difficult terrain, including rivers, flooded areas, and isolated villages, means health workers must travel by motorcycle, on foot, or by canoe to ensure no child is missed. Along the way, they not only administer preventive medicine but also educate families about malaria prevention.\u003C/p>\n\n\n\n\u003Cp>Their determination is critical. Without prompt treatment, malaria can rapidly progress from a mild illness to a severe, life-threatening disease, especially in young children. Untreated Plasmodium falciparum malaria can lead to severe anemia, cerebral malaria (marked by confusion, convulsions, or coma), breathing difficulties, organ failure, and death within 24 to 48 hours in the most serious cases.\u003C/p>\n\n\n\n\u003Cp>For children diagnosed with uncomplicated malaria, treatment typically involves artemisinin-based combination therapy (ACT), which is highly effective when taken correctly and on time. In Cameroon, treatment for children under five is provided free of charge in public health facilities, in line with national policy. This achievement was made possible by 12,237 distributors, 1,245 community supervisors, 524 area supervisors, and 138 district supervisors working across both regions.
