Throughout Indonesia, complications at birth continue to threaten the lives of women and their babies. The number of maternal and newborn deaths in Indonesia remains unacceptably high and, by some indications, has increased. From 2007 to 2012, for example, Indonesia’s estimated maternal mortality ratio increased from 228 to 359 deaths for every 100,000 births.\u003C/p>\n\n\n\n\u003Cp>These three women, fortunately, survived pregnancy and childbirth with the help of the Expanding Maternal and Neonatal Survival (EMAS) program that was launched by the government of Indonesia through the support of the U.S. Agency for International Development (USAID). EMAS, a program led by Jhpiego, works across the health system from village center to hospital to strengthen business practices, supervision and oversight of health service providers and the skills of individual midwives.\u003C/p>\n\n\n\n\u003Cp>The EMAS program helps train health workers to recognize and manage complicated labors like those Desi and Hamriani experienced. Twice a month, the midwives and nurses at Hamriani’s health center practice emergency drills to ensure that they’re always confident and ready to take action when the need arises. In fact, the very same day that Hamriani arrived at her health center with pre-eclampsia, midwives there had just completed a skill-building session on addressing pregnancy-related high blood pressure and its impact on mother and child. With skills fresh on their minds, the midwives worked together as a team, each managing a different aspect of Hamriani’s difficult labor. Through EMAS, an emergency text message-based referral system was in place, so the staff of the referral hospital was ready with the right blood type for transfusion, key supplies and the necessary intervention for Desi when she arrived.\u003C/p>\n\n\n\n\u003Cp>In Restiani’s case, her newborn son Edgar needed attention. He was taken into intensive care immediately after birth and stabilized. Midwives showed Restiani how to hold him skin-to-skin to transfer body warmth—an intervention called kangaroo mother care, which has been shown to increase a baby’s weight, prevent hypothermia and create a bond between parents and their newborn. Every day, Restiani swaddled Edgar against her chest. Many households in this region, including Jennifer and Ezekiel’s, face challenges such as food insecurity, income opportunities, and access to quality health services. It reached a point where even food was not guaranteed.”\u003C/p>\n\n\n\n\u003Cp>Jennifer and Ezekiel wanted to create greater financial stability for their family, with Jennifer hoping to contribute to the household income. But frequent, unplanned pregnancies made it difficult for her to pursue those opportunities, adding financial and emotional pressure to the family.\u003C/p>\n\n\n\n\u003Cp>Then one day, community health worker Abdul Athuman visited their home. Abdul had participated in a training led by \u003Ca href=\"https://tciurbanhealth.org/east-africa/\" target=\"_blank\" rel=\"noreferrer noopener\">The Challenge Initiative (TCI)\u003C/a>, a Jhpiego-led project. He also discussed a range of free health services available to the family, sharing information with Jennifer and Ezekiel that could help keep their children healthy.\u003C/p>\n\n\n\n\u003Cp>For Jennifer and Ezekiel, the decision to adopt a family planning method was not made lightly. After learning about their options, Jennifer decided to use a long-acting contraceptive implant that provides protection against pregnancy for up to three years.\u003C/p>\n\n\n\n\u003Cp>Being able to decide whether and when to have another child gave Jennifer greater peace of mind and allowed her to take on small income-generating work that contributed to the household income.\u003C/p>\n\n\n\n\u003Cp>While Jennifer received family planning services at the Buzuruga Health Facility, the couple was also able to access a broader network of health services available for free through TCI-supported outreach and integrated service delivery. Donald is one of the six providers from the facility who received training under the TCI project.\u003C/p>\n\n\n\n\u003Cp>Since 2019, the number of clients accessing family planning services at Buzuruga Health Center has increased from 1,134 clients to 5,521 clients in 2025—a 387 percent increase compared to the years preceding TCI interventions.
