Throughout her career, she has led strategic and evaluative initiatives that have shaped major global health programs and investments. Notable achievements include revising USAID’s maternal and child health investment frameworks to align with the Sustainable Development Goals, evaluating USAID’s flagship maternal and child health program, and designing a $1 billion successor strategy. She has also held leadership positions at Corus International and Abt Associates, managing global health and economic development portfolios across West Africa, francophone Central Africa, North Africa, and the Middle East. class=\"wp-block-paragraph\">San Isidro, Philippines — A single mother and a teacher, Vanessa shares a tiny house in the village of San Isidro in central Philippines with her two children, mother, stepfather and six younger brothers. And she is the happiest she has been in six years. After enduring years of emotional and verbal abuse from her husband, she found the courage to leave him and returned to her family home.   \u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">As she begins to talk about her marriage, Vanessa tears up.  “It’s just difficult to deal with the emotional bruises…because you don’t know how deep those are…. unlike the physical bruises that heal,” she says.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Vanessa met her husband while still in college, got pregnant and married in 2016, despite her mother’s objections.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">The couple soon moved to Cebu city, more than 300 miles from Vanessa’s family home: “It was the most isolated I’ve felt in my entire life. I (was) left alone in the apartment, just waiting for him to get back. I never got to meet any of his colleagues or their families. Coming from a poor family, I did not bring anything with me. He refused to buy me any clothes, not even a maternity dress. Her husband gave up his job, took out big loans in her name, forcing her to constantly borrow to pay the family bills.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“He would abuse me, call me ugly and chubby. He started cheating on me with other women and blamed me for it. I became paranoid. ‘What’s wrong with me? What should I do to make my marriage work,’” I wondered.  Every time he would get angry, he would smash something, like the computer I bought for work. He would control all my money. He didn’t want me to color my hair, paint my nails or wear lipstick. I was not allowed to wear jeans, not have a Facebook account, or meet my co-workers after working hours…he really, totally took control of my life.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“I kept telling myself it’s normal for a wife and husband to have arguments. She handed her a leaflet that explained abuse as per the Philippine’s Anti-Violence Against Women and their Children Act. “When I read the leaflet, I realized that my marriage was not normal, that I was being violated and it was unhealthy and damaging.”\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Roxas, a peer educator with the Sexual Health and Empowerment  (SHE) project, counsels  women on sexual and reproductive health and rights and gender-based violence and links them to care.  The SHE project is led by \u003Ca href=\"https://www.oxfam.ca/project/she/\">Oxfam Canada\u003C/a> and funded by Global Affairs Canada. Jhpiego’s role in the project is to improve the capacity of the public and private health systems to provide comprehensive and gender-responsive sexual and reproductive health services.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">When Vanessa’s husband slapped her during a  Christmas party thrown by her work colleagues, she decided to end to the marriage and take control of her life.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Roxas introduced her daughter to Mercy Amor Castillo, the Jhpiego-trained social worker at the local government of San Isidro. Like, if they would ask me for a burger or a pizza, I want to be able to give them that.”\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“Educating a woman about her rights is very important. In my case, I would not have stayed that long if I knew my rights from the very start,” says Vanessa.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">To women who are in abusive relationships, she advises: “\u003Cstrong>Find your worth. Don’t let anyone belittle you, say mean words to you…you don’t deserve it. class=\"wp-block-paragraph\">One year ago, in a small town in northwest Sierra Leone, Catherine Kamara, a 24-year-old pregnant woman, tested positive for HIV during her antenatal care appointment.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“The news of my health condition was overwhelming and scary,” says Catherine. “With counseling and encouragement from Nurse Tinny, I discussed it with my husband and asked him to get tested. My husband’s test was positive while my firstborn tested negative. This news brought so many unsaid words between us for a very long time. In addition, access to HIV testing and treatment is limited and viral suppression among children is low. The country is lagging behind its \u003Ca href=\"https://www.unaids.org/en/regionscountries/countries/sierraleone\">UNAIDS 95-95-95\u003C/a> goals for children—25% of children living with HIV have been diagnosed, 25% are receiving \u003Ca href=\"https://hivinfo.nih.gov/understanding-hiv/fact-sheets/hiv-treatment-basics\">antiretroviral therapy (ART)\u003C/a>, and 18% have reached viral load suppression. Among the eight countries in the West Africa Regional Program of the U.S. \u003Ca href=\"https://www.state.gov/pepfar/\">President’s Emergency Plan for AIDS Relief (PEPFAR)\u003C/a>, Sierra Leone has the lowest HIV treatment coverage for children.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Funded by PEPFAR and administered by the \u003Ca href=\"https://www.hrsa.gov/grants/find-funding/HRSA-21-144\">Health Resources and Services Administration\u003C/a>, \u003Ca href=\"https://www.jhpiego.org/countries-we-support/sierra-leone/\">Global Reach II is a five-year project operating in Sierra Leone\u003C/a>, Zambia, Uganda and Ethiopia that helps address challenges in country-level HIV responses. In Sierra Leone, the project strengthens health care providers’ skills to ensure HIV services are accessible, high quality and integrated in all areas, including antenatal care.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">For Catherine, the young mother in Sierra Leone, support from Nurse Tinny, who specializes in maternal and child health, was essential to managing her HIV and making sure she didn’t transmit it to her son. Maternal and child health aides work closely with each pregnant woman or mother who comes to the facility to understand their unique challenges in adhering to treatment, such as side effects or difficulty with medication schedules. Nurse Tinny talked to Catherine about the importance of regularly taking her medication, eating a balanced diet and attending antenatal clinics.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“I needed to deliver a healthy baby and there was no other way of doing that than taking my medicine and following the recommended guidelines for HIV-positive pregnant women,” Catherine says.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Catherine gave birth to a healthy baby boy, overcame her hesitation in taking medication while breastfeeding, and consistently adhered to the ART regimen. As a result, she created a unique bond that brought her and her baby closer together than she could have imagined possible while having HIV.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“I can’t change the past, but I can improve my future by living healthily and supporting my children in the best way possible—just like others living with HIV are doing,” says Catherine.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">For Nurse Tinny and other Global Reach II-trained health care providers, the goal is to support clients to achieve a level of HIV at which the virus is undetectable and therefore, no longer transmissible. “Achieving undetectable viral load levels is a journey, and we are on that road,” says Nurse Tinny. “HIV is not the end. My mother-in-law urged me to go to the hospital. I went and did a couple of tests. The pregnancy test was positive. We were so happy because we wanted to start a family,” said 28-year-old Linda.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">When she was six weeks pregnant, Linda was introduced to the group antenatal care (G-ANC) sessions at Kangundo Hospital in Kenya. G-ANC brings together a group of women to learn, share and experience pregnancy together. Meeting monthly with the same group fosters friendships and support and improves the experience for women. “This was my first pregnancy and I knew nothing. I went ahead and attended the group sessions,” she said.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">\u003Ca href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7939754/\">Linda attended all G-ANC sessions and her husband noticed a difference\u003C/a>\u003Ca href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7939754/\" target=\"_blank\" rel=\"noreferrer noopener\">.\u003C/a>\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“She would be singing while doing her chores . . . it was a way of memorizing what they were learning,” he said. “Linda shared her notes with me after every class. We were moving step by step together. I saw the benefits of the classes and supported her to attend without fail.”\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“I wanted to get as much information to prepare me for the pregnancy and delivery,“ Linda explained. “My favorite topics were how to observe problems, danger signs and taking care of myself. One time, I noticed some discharge from my breasts. My friend told me it’s a miscarriage. During my next class, I wrote my question and dropped it in the wonder basket. The facilitating nurse-midwife privately helps women with low literacy skills to add their questions, and she engages the group to help answer the questions at the end of the meeting.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">\u003Ca href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7939754/#:~:text=WHAT%20IS%20GROUP%20ANTENATAL%20CARE,centre%20at%20the%20same%20time.\">G-ANC is associated with a higher proportion of women receiving quality services, including more antenatal care checkups\u003C/a>\u003Ca href=\"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC7939754/#:~:text=WHAT%20IS%20GROUP%20ANTENATAL%20CARE,centre%20at%20the%20same%20time.\" target=\"_blank\" rel=\"noreferrer noopener\">.\u003C/a> Women also report a positive experience in G-ANC, sharing that they were able to ask questions and learned important things about preparing themselves for childbirth and post-birth and keeping their baby healthy.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">They also reported forming meaningful relationships and a spirit of open communication with their health care providers.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Pauline Muema, a nurse at Kangundo Hospital, remembers Linda’s engagement with the other women in the group.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“Linda asked questions, oriented [other pregnant women as they] joined the group, and she became the group leader,” said nurse Pauline. “She delivered a healthy baby girl in our health facility. Baby Bella is four months old. We walked together during her pregnancy and now we are monitoring Bella as she grows. “\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Kangundo Hospital was one of the first facilities in Kenya to introduce G-ANC, during a Jhpiego research study on G-ANC in 2016, and staff have remained enthusiastic about the approach. In 2021, the Machakos County Health Management Team selected Kangundo as one of 12 facilities to adopt G-ANC as the routine standard of care so that all pregnant women may benefit. County managers partnered with Jhpiego to scale up G-ANC and learn together about what is needed to make and sustain this change, so other facilities and counties throughout Kenya can more easily switch to G-ANC. She has been a champion since Kangundo started offering G-ANC. Through member-guided group discussions, traditional aspects of antenatal assessment such as clinical checks are blended with information sharing and peer support.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“With one-on-one ANC sessions, there are no targeted messages. But with group ANC we have a flow [of information],” said nurse Pauline. “Sometimes a mother may walk into the clinic and she is 28 weeks [pregnant]—where would you start? With the group ANC we are able to have time and link the moms to each other for support.”\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">To date, Pauline has formed and facilitated 80 groups, hosting 500 meetings and reaching more than 1,400 moms at Kangundo Hospital. Cajoled by her friends to tag along as they took the entry exam for nursing school, Masri decided to take the test herself—and was the only one among her friends to pass it. That paved the way for a career in midwifery that would eventually lead her to a position managing a community health care system.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">As the leader of the Batakte Community Health System, she oversees the delivery of primary health care to around 20,000 Indonesians in the West Kupang subdistrict. Using her almost three decades of experience as a midwife, Masri leads 80-plus primary health care workers—from general practitioners and dentists to midwives and nurses—who work in 54 community-based health posts and outreach centers, including the main community health center, smaller village health centers, one-room maternity huts and integrated service posts that are scattered across the region.\u003C/p>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">From Village Midwife to Health System Leader\u003C/h3>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Masri started her career as a village midwife, attending to pregnant women, mothers and babies, preparing for safe and healthy births and delivering quality postnatal care. Now, she spends her days making sure the 54 community-based health posts and outreach centers that are part of the Batakte Community Health System provide quality health services to all who need them—such as the steady trickle of more than 2,500 clients who visit the Batakte Community Health Center each month (10–15 of whom are women coming to the facility to give birth). Client satisfaction is key for her.\u003C/p>\n\n\n\n\u003Cblockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n\u003Cp class=\"wp-block-paragraph\">\u003Cstrong>The educational background and experiences I have had as a midwife help me understand issues from both sides. It makes me feel like there’s a calling for me to make sure this health center provides satisfactory services to the clients.”\u003C/strong> – Masri Ndoen\u003C/p>\n\u003C/blockquote>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">The Indonesian government is helping her do that. As part of a new focus on strengthening the country’s primary health care services, it has identified two main areas of focus for health centers like Masri’s: facility-based care and community-based care.\u003C/p>\n\n\n\n\u003Ch3 class=\"wp-block-heading\">Morning Management Is Key\u003C/h3>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Masri’s mornings are often dedicated to the community health center. By 7:30 a.m., she’s already started her 20-minute drive to the center to arrive for a daily briefing with her team. Then she checks in on clients and different service providers, hearing about any triumphs or challenges they have experienced. from her team that they needed more support. Every Friday she creates space for the community to come together and talk about their health, whether at sports events, breakfasts with different villages and schools, or neighborhood clean-up activities. Region of Tanzania. She and her small team work together to make sure every person who visits—ranging from expectant mothers, children, and the elderly—receives quality primary health care. But she’s not only a health worker, she’s a neighbor, living a few hundred feet from the facility and from the clients she serves every day.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">Today, she’s at her health facility and out in the community meeting with some of her neighbors, including two women who have questions about family planning, a family at a nearby farm who rely on Jalia for their communities are the health workers who can help us achieve Universal Health Coverage. With a strong health system to back her up, Jalia strives to ensure equitable access to essential health services for everyone in her community, making sure that their health and well-being are not a privilege for a few but a right for all.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">That means providing a wide range of care tailored to the individual needs of each client she sees, including maternal and newborn health services, family planning, HIV testing, vaccinations and tuberculosis screening.\u003C/p>\n\n\n\n\u003Cp class=\"wp-block-paragraph\">“I live in the village and am very friendly, which makes it easier for clients to speak out and be more open when they are sharing information about their health,” she says. “I can tell the community really enjoys me serving them. One woman is pregnant and would like to explore postpartum family planning once she gives birth. As she continuously hones her skills, she also tries to stay up to date on the health of her community by working closely with a diverse group from community health workers and lab technicians to medical attendants and nurses.
