Safe motherhood begins with high-quality maternal health services, which are often challenging to provide in Malawi. Although maternal deaths have been steadily decreasing, there are still 349 deaths per 100,000 deliveries in the country. To ensure a happy and healthy delivery day, the Government of Malawi recommends that pregnant women attend eight antenatal visits at a health facility, in line with World Health Organization (WHO) recommendations. But few pregnant women meet this target, due to a range of barriers to care.

Like Pauline, health workers in Malawi’s Blantyre region struggle to provide comprehensive high-quality care to pregnant women, due to gaps in staffing, skills and equipment. Pauline works at the only government health center in Zingwangwa, Malawi, which serves almost 140,000 people. Her day at the facility starts as early as 7 a.m., when she is welcomed by a waiting queue of pregnant women.

“Every day, I serve at least 20 pregnant women and we have a minimum of five births. This seemed to be a hard-to-realize dream for most health centers, including the one where Pauline works, because ultrasounds aren’t routinely available at the health center level, and health workers don’t have the skills to use them.

Nurse-midwife Pauline Jalisi. Through a collaboration between Jhpiego and Johns Hopkins Research Project – Blantyre as part of the Antenatal/Postnatal Research Collective (ARC), and in partnership with the Ministry of Health in Malawi, Pauline and two other midwives from the Zingwangwa Health Center joined 41 other midwives at a training on new service delivery technologies to increase continuity of care and improve quality of care.

ARC collaborated with hundreds of women, their families, providers and community members in Ethiopia, Kenya, Malawi and Mali to explore service delivery approaches that provide safe, respectful and valuable experiences for women and their health care providers. In Malawi, ARC designed a study to explore the use of a portable ultrasound device called Butterfly iQ during antenatal care (ANC) services at the health facility and whether midwives could be trained efficiently to use the device.

“This is the most extensive study of feasibility of ultrasound introduction in Malawi to date,” says Dr. Lisa Noguchi, Jhpiego’s Director of Maternal Health. “The study directly addresses WHO’s call for further implementation research on introduction of ultrasound in ANC.”

The Butterfly iQ study was implemented in partnership with the Malawi Ministry of Health in 2021–2022 and the University of Malawi College of Medicine with funding from the Bill & Melinda Gates Foundation.

Now, Pauline can conduct ultrasounds for pregnant women and her work has contributed to more effective collaboration with the local hospital. She is happy she can provide a more positive pregnancy experience for the many women from her area.

“Our quality of care at this community facility has been enhanced through the provision of ultrasound scanning to all pregnant women that come for the antenatal clinic,” she says. “Ever since I was trained, I have scanned over 100 women and referred those with complications to the referral hospital in good time. It brings so much joy to see the women happy to see their unborn babies and even hear their heartbeat. It’s something they have never experienced before.”

At least 44 midwives from 10 health centers within Blantyre City and surrounding areas were trained to use the Butterfly iQ device by seven medical doctors from the Queen Elizabeth Central Hospital’s obstetrics and gynecology department. Over the course of the study, the health centers conducted 1,500 ultrasounds, and continue to offer them today.

“I’m so happy that all centers managed to do successfully offer ultrasound scans, says Pauline. “[Even after the ARC study] midwives continue to provide ultrasounds to pregnant women in their health centers.”

“We are delighted to see that the midwives have made a difference in the lives of over a thousand women in this short time and will continue to do so,” said Fidelis Sindani, Jhpiego’s Monitoring and Evaluation Director in Malawi. “The study has underscored the role of midwives in offering comprehensive, high-quality maternal Malaria Dashboard Enhances Data Use for Decision-Making","

Abdallah Hamisi noticed a troubling trend at Raha Leo health center in Zanzibar, where he has been a laboratory technician for nine years. He is responsible for identifying the presence of malaria infection in patient blood samples and providing that information to health care providers so they can decide what treatment to offer to help patients recover quickly. When malaria-related data collection and reporting issues are identified but not properly tracked over time, it can lead to a pattern of inaccurate decision-making due to under- or over-reporting of disease burden.

To address these issues, the U.S. President’s Malaria Initiative (PMI) Impact Malaria Project collaborated with other implementing partners and the Zanzibar Malaria Elimination Programme (ZAMEP), under the Ministry of Health, to build and construct an interactive dashboard. This dashboard introduced high-quality data visualizations and assisted Zanzibar in decision-making regarding malaria-related interventions.

Deployed in March 2022, the malaria dashboard portal comprises data from the weekly electronic integrated disease surveillance and response, the malaria service data quality improvement tool, distribution of insecticide-treated mosquito bed nets, malaria case notification and indoor residual spraying.

Hamisi noted an immediate difference with this tool. When one teammate “enters information into the system, anyone else on the team can easily access the information and use it for further planning,” Hamisi said, highlighting the increased access to data that the dashboard provides. “The availability of an interactive dashboard aids in the timely visualization of data, which also helps in decision-making. When the facility testing rate was low, we could identify a pattern through the data and address it promptly.” If a decision-making error was identified, the facility would conduct supportive supervision visits to ensure testing and diagnosis accuracy so that patients received the high-quality care they needed quickly.

Abdallah Hamisi uses an interactive dashboard for supportive supervision at Raha Leo health center in Zanzibar.

In areas of Zanzibar that are close to eliminating the malaria burden, data tracking and accuracy are critical to reaching zero malaria cases. Juma H. Khamis, the information technology officer at ZAMEP, also noticed the value of the tool across teams within a health facility and sees the application as a stepping stone to making technological improvements in the collection of malaria data toward the elimination goal.

“The great thing about this application is that any department may access the data at any time and rapidly act upon it when necessary,” he said. He noticed the departments that have benefited the most from the dashboard are those dealing with monitoring and evaluation, diagnostics, treatment and logistics.

Khamis noticed that the dashboard also highlighted underutilization of the antimalarial medicine primaquine at the private health facilities, which prompted ZAMEP to facilitate the development of malaria management job aids and antimalarial record books. Keeping essential malaria medicines stocked, and properly using supplies, is important to be able to provide the community with quality services. This shift in health policy aided the private facilities in better adhere to the national malaria treatment guidelines in the pre-elimination areas.

“In general, ZAMEP is very grateful for the PMI Impact Malaria efforts in supporting development of the interactive dashboard. The dashboard will further improve the quality of data reporting and visualization, which is very important for ZAMEP in documenting and monitoring progress towards malaria elimination in Zanzibar.” — Juma H. As the International Health Regulations (IHR) (2005) outline, the point is to prevent the international spread of diseases. [The IHR (2005) is a legally binding agreement among 196 countries that defines how countries should respond to public health emergencies of international concern.]

Q: How does health security at land borders differ from health security at airports or seaports?

IS: The vast majority of travelers are crossing through the land borders—at both designated [those that meet IHR core capacities] and undesignated ground crossing points—not the airport. Only people who have money can afford to fly. Ground crossing points should get more attention when it comes to response. The risk of spread is evident from the Ebola outbreak in 2014/2015. The outbreak started in Guinea and affected all the neighboring countries because the communities are so interconnected, with vast population movement. The majority of people who pass through the crossing points live within the border communities, especially those informal crossing points that are not manned. So it’s necessary to intensify surveillance in the nearby health facilities—and ensure the supply of medical consumables.

Q: What are the main issues to consider when developing a border security plan?

IS: For border health generally, we are governed by the International Health Regulations. At the same time, we try to tailor the response plan to the locally accepted way of doing things. Then, of course, communication is key. You need to communicate to travelers why you’re doing this, what are the risks, what diseases are endemic in the visiting country. And there should be community engagement with border communities, because in some places people are crossing without [border] personnel for screening. Community stakeholders can play a role in supporting these unmanned crossing points.

Q: How do you prepare health workers and officials from different countries to work together and respond to an actual health emergency?

IS: You have to maintain collaboration over time. If there’s strong collaboration between countries, maybe through a formal agreement and information-sharing mechanism, then there can be a joint response because they’re used to doing things together. We initiated some level of collaboration during a regional tabletop exercise [a desktop simulation of an emergency response, such as a Lassa fever outbreak]. Our colleagues from Liberia and Guinea came to act as observers and evaluators, and we also went to Liberia to do the same. We’ve built some connections. To sustain that connection, we have to continue to do activities together. We’ve had regional meetings, as well, with the four countries in the Mano River Union region (Liberia, Guinea, Sierra Leone and Côte d’Ivoire) and binational meetings with Liberia.

We need joint cross-country plans, like the one we developed for Sierra Leone and Liberia during one of our cross-border meetings. Prediction and prevention of outbreaks will only be relevant if countries respond as one.

Every outbreak is a lesson. After the Ebola outbreak, diverse country coordination has been strengthened. Countries have learned how to mount an effective response to public health emergencies. The COVID-19 pandemic also provided an opportunity for the health security to be strengthened, despite language differences. For instance, Guinea has had recent disease outbreaks (especially viral hemorrhagic fevers) in the region. And they didn’t disappoint. They shared a lot of best practices. That’s the idea of these exercises—simulating a scenario and everybody gets to talk about how they would respond.

You can never stop preparing. It’s not cheap, but if you prepare very well, it will save you a lot more money if there is a pandemic or outbreak—and not just money, but lives lost. Preparedness helps reduce the money you’re spending when there’s an emergency. That’s why we have partnerships like The Pandemic Fund. The World Bank, Global Fund, everybody now wants to invest in preparedness.

Q: What other actions are you taking to support border health?

IS: Something we are doing uniquely here is population movement mapping in land border districts to identify areas with risk of disease transmission. This is a mixed-method study with qualitative focus group discussions and spatial analysis to identify where people frequent, including their point of entry and the reason for the visit. It could be for health care, for religious purposes, for schooling, or for markets. When we identify which areas are visited the most, we plan in terms of what to do if there’s an outbreak in the neighboring country, and make plans to allocate resources accordingly. During preparedness or silent periods, we can recommend that those places be strengthened for surveillance as well as risk communication. You can strengthen surveillance at the health facility. You can strengthen risk communication at the schools, at the market and at religious sites. You can also target the health facility for increased infection prevention supplies because they may be at more risk.

We are also supporting the Ministry of Health to develop a national border health policy. While the public health emergency response plan is meant for emergency settings, the border policy will guide both routine and emergency settings. The policy, based on the International Health Regulations, will detail the governance, components of the port health system and roles and responsibilities of the different sectors working in the national border health system. I don’t think any other country has this policy. We need to raise the bar.

","
As a medical doctor with training and hands-on experience in infectious disease surveillance and emergency response, Ibrahim Seriki is the expert you He understands the local health system and the challenges faced by health workers in the region.

Collecting, processing and analyzing health data has been a daunting task for facilities and health workers, especially in the fight against malaria. Accurate data collection includes tracking and keeping stock of essential supplies, such as antimalarial medicines, treatments to prevent pregnant women from getting malaria and insecticide-treated bed nets, which protect against malaria-carrying mosquitos. This data is important to ensure that facilities can reliably serve their communities.

The data received from the health facilities and district levels had been of low quality…. During facility-based visits, Joseph and his team shared their knowledge and tools to effectively collect, analyze and use data to identify gaps in malaria prevention and treatment. They also coached the trainees on how to use data to inform decision-making and prioritize interventions that would have the most impact. In all, they reached 90 health facilities and trained 232 health workers.

In May 2021, Joseph and his team trained health information officers and other frontline staff at the Prampram Community Polyclinic on data quality and visualization.
