Malaria, One Woman and One Bed Net at a Time","

In Liberia, a country where malaria transmission occurs throughout the year, the Ministry of Health prioritizes the provision of insecticide-treated bed nets (ITNs) to pregnant women during the course of their pregnancy. ITNs provide effective protection against malaria and have been shown to reduce malaria illness and death in high-burden settings. These protective benefits are especially important for pregnant women, not only because they are at greater risk of malaria infection and symptomatic malaria disease than non-pregnant adults, but because malaria in pregnancy contributes to maternal and newborn morbidity and mortality.

Despite the Ministry of Health’s commitment, access to and use of ITNs has been relatively low, with only a little over half of pregnant women receiving ITNs during their antenatal care in 2018. Reasons for this low access and use include low awareness of the benefits of ITNs and of the need to sleep under one every night.

Hawa Johnson poses for a picture with her new bed net at Dagweh Community Clinic. Photo by Karel Prinsloo for Jhpiego.

Through the Strategic Technical Assistance for Improved Health Systems Performance and Health Outcomes (STAIP) project, funded by the U.S. Agency for International Development, Jhpiego has been supporting the Ministry of Health to ensure improved access to ITNs for pregnant women. The project does this by providing job aids that remind health workers to offer bed nets to pregnant women and to clearly communicate the benefits of ITNs, the importance of using them every night and how to care for them. It also provides health care workers with in-service training, supportive supervision and continual coaching and mentoring on the national guidelines for malaria in pregnancy.

As a result, pregnant women like 24-year old Hawa Johnson, mother of four, are receiving ITNs when they go for their first antenatal care visit. Johnson visited Dagweh Clinic in Bomi County in the northwestern part of Liberia, one of the 400 health facilities supported by Jhpiego in Liberia.

“I visited the clinic,” she said, “and they gave me a check-up, and told me I was pregnant, so they gave me a mosquito net for me to sleep under to prevent myself and my unborn child from getting malaria…. I do not have malaria. I sleep sound at night. I do not have any sickness in my body… I have not had malaria because I always sleep under the net, every night. To take care of the mosquito net, I wash it with bathing soap once a month, hang it up, and when it dries, I hang it back up above my bed before I sleep under it.”

Efforts such as these are having an effect, one woman at a time. In just three years, the STAIP project’s interventions have contributed to an increase in the percentage of pregnant women who received ITNs during their antenatal care in Liberia from 57% in 2018 to 74% in 2021. Fortunately, women like Johnson are getting the protection that the Ministry of Health intended.

","
In Liberia, the Ministry of Health prioritizes the provision of But this is just one of many misconceptions about self-care that exist: that midwives and other health care providers are opposed to individuals, families and communities preventing disease and coping with illness with or without their support. Not true. And neither are a whole host of common misconceptions, such as:

Self-care is just for patients.

The facts: More than half of midwives surveyed in Rwanda reported that self-care is for patients—and not for them. The mindset that says, “I should be able to cope. . . I don’t have time for self-care. . . I need to do other things first” comes at a cost. A review of care providers in sub-Saharan Africa reveals midwives experience high rates of burnout, along with emotional exhaustion and other physical issues.

Health care providers need to take their own health off the backburner and walk the talk by practicing self-care in addition to promoting it to patients. Self-care should be integrated with occupational health and safety in workplaces. Employee wellness programs can also address self-care by health care providers.

Taking care of ourselves is selfish and takes too much time.

The facts: The survey of Rwandan midwives reveals that 79% of respondents don’t have time to take care of themselves. Women comprise just 10% of the representatives of the U.S. Coronavirus Task Force and 20% of the WHO Emergency Committee on COVID-19.2 A global survey of 30 countries found that women represent an average of 24% of national-level decision-making bodies on COVID-19.3 By failing to include women’s voices, we not only lose critical technical expertise, but also increase the risk of implementing policies that fail to account for the gender dimensions of health emergencies.2 Losing gains in access to sexual and reproductive health services, overlooking negative consequences of epidemics on maternal and child health, failing to address the increased risk for gender-based violence—specifically intimate partner violence—due to lock-downs, and limiting conversations on economic recovery to the formal sector are just some of the consequences of gender-blind policies that do not also consider the intersectional needs of women.2–4 (Intersectionality recognizes that women are not a homogenous group, and that gender interacts with other determinants like age, geographical location, sexuality, class, religion, ethnicity, citizenship, and disability to shape access to services and health outcomes.5)

Given this backdrop, there is no better time than now to invest in gender-transformative leadership to promote gender equality in health organizations and, more broadly, in the communities where they work.6 (Gender-transformative leadership development offers deliberate consideration of both formal and informal gender power structures and discriminatory practices.6) To address factors contributing to the global gender gap in leadership, Jhpiego’s recently launched Gender Transformation for Health: A Toolkit for Health Workers covers topics like global disparities in women’s leadership, gender as a determinant of career advancement, gendered patterns of communication, mentorship and sponsorship, and linkages between power, gender norms, and sexual harassment.

The toolkit reflects numerous calls to action for equitable representation of women in global health decision-making. The strategies shared in the toolkit’s modules apply an intersectional lens and complement existing leadership curricula, providing tools to health care workers and managers to advance gender equality. The approach is particularly relevant in the context of today’s pandemic, but it can also be used long-term to spur greater inclusion of women’s voices in global health.

See the toolkit

References:

World Health Organization. region.

Tesfaye Dadi, Human Resources Officer for the Oromia Regional Health Bureau (RHB), typed a series of keys on his laptop before he looked up and smiled. His desk, and those of his other colleagues in the expansive well-lit room, looked tidy and spotless. Documents were neatly stacked in lock-and-key shelves. A networked Hewlett-Packard printer was accessible on one of the shared tables. He was eager to get to work.

But Oromia—one of the largest RHBs in Ethiopia serving tens of thousands of health workers—has not always functioned so well in the 7 years Dadi has worked here.

“All offices at the RHB were congested. The paper files and office furniture were not properly arranged, making it difficult to clean, and there was a poorly functioning air conditioning system,” Dadi said. He recalled the previously overcrowded and poorly equipped office, which led to time wasted and poor customer service for the health workers who often traveled hundreds of kilometers to the bureau to process paperwork for recruitment, transfer and promotion. The RHB office also manages licensing and registration, educational opportunities and capacity-building of health workers from across the region—including zonal health departments, district health offices and health facilities.

“Employees faced difficulty to get their own personnel files, and sometimes they incurred additional cost and time to get access to different human resource management services at the regional bureau,” Dadi added. “There were also budget and capacity limitations to conduct the planned activities according to schedule and outcome.”

Understanding the importance of quality services to health workers whose time would best be spent saving lives rather than chasing employment files at the RHB, Dadi and his colleagues teamed up with the Strengthening Human Resources for Health (HRH) Project, funded by the U.S. Agency for International Development (USAID), to spearhead efforts to improve Oromia RHB’s productivity and ability to provide timely customer-oriented and employee-friendly human resource management services.

Dadi and his team, with support from the HRH Project, followed the lead of the Ethiopian government and adopted the kaizen workspace organization philosophy—a Japanese business improvement practice—to identify the needs and gaps in their area. They then developed a plan of action, which was later endorsed by RHB management. This was followed by the training of 86 staff—management officials, middle-level managers and experts from all work units at the RHB—between February 2015 and June 2016 on how to apply kaizen principles. The team then applied the principles, followed up on activities and provided coaching and mentoring to improve the working environment.

“Recognizing that the work environment is an important determinant of job satisfaction, motivation and productivity, the government of Ethiopia introduced kaizen in the public sector to make work spaces pleasant. Since the HRH Project is tasked to improve motivation and retention of health workers, it supported the government’s efforts to improve work climate,” Dr. Tegbar Yigzaw, HRH Project Deputy Chief of Party, noted.

The efforts have paid off and Dadi has every reason to smile as the “RHB is a much more pleasant working environment,” he said buoyantly. The seating arrangements have been adjusted to meet staff needs and offer the best customer service experience. New furniture and metal sliding shelves for storing records were procured; and a fresh coat of paint was applied. Staff members no longer complain about the lack of fresh air.
